The reporting period is from July 1 through June 30.
Executive Order 96-9 requires State agencies to document compliance with Titles I and II of the Americans with Disabilities Act, and submit a report to the agency head and the state ADA Coordinator.
The report must be submitted online by the first business day in September. Please answer the questions regarding your agency’s efforts toward increasing awareness and accessibility for people with disabilities. Once submitted, your responses will be transmitted directly to the survey database maintained at DOER. Use your computer mouse and keyboard to answer questions as they appear on your screen. Be sure to print a copy of the report for your records before submitting it. Click the submit button at the end of the survey.
